By submitting this MSBOA District 5 Junior High All-Star Weekend Nomination form, the director agrees to work on Saturday and be in attendance for the entire All-Star Weekend. In the event a director does not fulfill their work assignment, their students' (school) will not be allowed to enter students in the All-Star Weekend the following year.

By submitting this form, we hereby certify that all students nominated are bonafide members in the entering school's instrumental music program. After submitting this form, you will receive an email at the address specified above within 48 hours (2 business days). You must print this email, sign it and have your building Administrator sign it and mail it, with $50 payment to:
